If you have an initial appointment, I will send you an invitation to set up a portal through TherapyNotes, a HIPAA compliant telehealth platform I use for my records and telehealth sessions. Once you set up a portal, you will see links in it for all of the forms to complete. You will also be able to upload your insurance card(s). This is all electronic and should be completed prior to the first appointment. Please note that the portal you set up will be how you access all telehealth sessions, how you can communicate confidentially with me and how you will receive any future forms or assessments.
